Inflatable/Bounce House Permission Application

  2. This form is to be filled out by any resident/person that would like to bring an inflatable to a City Park. Bouncers are not permitted to be at the Westlake Village Community Park. Please note that only one standard size bounce house per party is permitted (no obstacle courses, water slides, etc.) and that the City requires this form, liability insurance, and a separate additional insured endorsement (both naming the City of Westlake Village). By submitting this form you are acknowledging that you have read the inflatable rules and regulations and will comply with them.
  3. Please give us the month, day, date, and year that you are requesting to place your bouncer.
  4. Please give us the range of time that you will have the bouncer at the park. Include set-up and take down time.
  5. Please list the name of the bouncer company that you will be using.
  6. By clicking next to each rule, you are acknowledging that you, the renter, and the bounce house company understand the rules and regulations of operation at a City park.*
  7. Please upload the Liability Insurance Form from the bouncer company using the required additional insured language listed at the top of this application.
  8. Please upload the Separate Additional Insured Endorsement using the required additional insured language listed at the top of this application.
  9. Please submit the contract that you the user have with the bounce company for the rental.
